PRACTICE ACTIONS ON THEIR PRIORITIES FOR 2014/15
We are pleased to say the Practice is up and running with its new ecg service and both Nurses and 3 Health Care Assistants have been trained to use the equipment.
The Practice now provides child asthma appointments that are 30 minutes long to allow the Practice nurse to discuss a detailed care plan with both our young patients and their parents.
There are now HCA appointments available at 7am on a Wednesday and a Thursday together with nurse appointments on a Wednesday. On reviewing March’s appointments, the nurse’s appointments have been fully booked however the HCA appointments are slightly more mixed. We will continue to monitor this as it might be that they are not well publicised enough at the moment. The early appointments allow our patients to have fasting blood tests early without having to go without food for too long, we also hope we are providing appointments for our patients that find it difficult to come in during normal surgery hours.
Our copd and diabetic patients have seen their appointment times double to allow for a comprehensive discussion with regard to their care.
The dementia screening service is also up and running with our Practice Nurses this allows patients to have an appointment within the practice, which is more convenient for our patients and allows patients to discuss any concerns that they may have had in a familiar setting.
We have increased the number of HCA appointments purposely to allow for more NHS health checks to be available and the Practice is writing to patients themselves to encourage patients to take up this screening.
The Practice Nurses have been visiting our housebound patients to regularly review their care and this we feel has been a real benefit to our patients.
All our over 65 patients we have attempted to call at least once to invite them in for their flu vaccination and if they have never had an NHS health check we have tried to accommodate this at the same time.
Chronic Kidney Disease screening is now up and running by all our HCA’s and Practice Nurses.
You may by now have used our electronic prescribing system which is now up and running. The first few days did take a little longer for our patients to order their prescriptions only because we need to make sure a nominated pharmacy is in place ECT but things are settling down.
I am still awaiting news of our telephone system so unfortunately I am unable to update you on that just at the moment.
