If you live within our catchment area we have registration forms available at Reception. Please complete these forms this will enable us to have as much information about you and your care before we receive you full medical records from NHS England.
We ask for proof of identity and address, this is to ensure we are protecting your data and is used to check your details match with the information held on the NHS central patient registry. This will ensure that your previous medical notes are passed on to our practice.
If you are registering a child please bring with you your child's birth certificate.
